Please review all of the instructions and information for completing forms before starting the form process.  It is required to complete an evaluation form for each day you attend the conference and a CEU attestation form BEFORE you download and print your CEU certificate.

Please complete a session evaluation form for EACH DAY you attend the conference at the end of the day.

When you open the forms, scroll towards the bottom where your email address and name will be preloaded.  Click "Next".  It will then go through each session of the day.  If you attend, click "Yes" and a series of feedback questions will populate.  Complete these questions and then click "Save and Next".  If you did not attend, click "No" and it will move onto the next session.  After the final session click "Save and Finalize".  A popup window will appear asking you to confirm you would like to submit.  Press "Ok" to submit.

Please complete the CEU attestation form when you have completed your LAST DAY of the conference.

When you open the form, complete your information on the first page and click "Save and Next".  On the next pages, please enter your initials next to any sessions that you attended.  At the bottom of the page, enter the total number of CEUs you earned.  If you did not attend any sessions, please enter 0.  Please enter information for all three days of the conference (even if you did not attend all three days).  On the final page, total your CEUs fort the entire conference as well as the ethics CEUs earned.  and you can submit.  When finished click "Save and Finalize".

You will receive an email with the information from your completed attestation form that you can print.  A link to download and print your CEU certificate will also be included.

Once you have completed your evalution forms for each day you attended the conference and completed the CEU attestation form you can print/save a copy of the CEU certificate.  Click the link the below to open the form in another window where you can print/save a copy.

CEU Certificate - Updated as of 10/22/2021 with corrected information that now says 18 CEUs.  Please print a new copy if you previously printed a copy that lists 15 CEUs on the bottom.
