Pediatric Transplant Social Work 101 (and 102 and beyond!)
The role of the pediatric transplant social worker is complex and vital to a robust pediatric transplant program. As pediatric social workers, we are tasked with using a family-centered approach and work with the patient and their family throughout their course. The social worker is often the bridge between the patient/family and the medical team and our role includes communicating the strengths, needs, and health literacy of the family to the team. The pediatric transplant social worker is tasked with assessing safety, adherence, and social needs in every interaction. Due to the size of our pediatric programs, the relationship between the social worker and patient/family spans all phases of the transplant process; namely, pre-transplant, evaluation, post-transplant care in both the inpatient and outpatient settings through transition to the adult transplant center or natural death.
During this workshop, we will review the comprehensive Pediatric Transplant Psychosocial Assessment Tool and the role of the social worker on the transplant team. This assessment examines all areas unique to the pediatric transplant population, including those psychosocial factors that could impact post-transplant outcomes. We will review ways for social workers to position themselves as subject matter experts within their multi-disciplinary team. We will also draw upon the experience of other pediatric transplant social workers in the room to increase support and collaboration across the country. Examples of a pediatric psychosocial evaluation, documentation of psychosocial concerns, and a list of expectations for parents of children awaiting transplant at home will be provided.
